Removing your horse's data

Please complete the form below to remove your horse's data from the Equine Veterinary Surveillance Network.

  • To remove data for all the horses you own, enter your unique client ID number and leave the horse's unique ID number blank
  • To remove data for specific horses only, please enter your unique client ID number and each unique horse ID number, separated by a comma if more than one.
