Copies of Occupational Health records
Occupational Health (OH) records contain sensitive, confidential, personal information. These records can only be disclosed with written consent from the data subject.
Requests for copies of OH records must be made by the data subject, or with written consent from the data subject, in accordance with section 7 of the Data Protection Act.
Requests for copies of OH records can be made via the University online store.

In accordance with the Data Protection Act a statutory fee of £10 is payable for each copy.
Cheques should be payable to "The University of Liverpool".
When requesting copies of OH records, please confirm your full name, date of birth, student or staff number and the address to which the records should be posted or emailed, to ensure data accuracy and security.
