Requests for copies of OH records or immunisation and screening records must be made by the data subject, or with written consent from the data subject, in accordance with GDPR.
Requests can be made via the University's online store.

When requesting your records, please confirm:
- Your full name
- Date of birth
- Student or staff number
- Address the records should be posted or emailed, to ensure data accuracy and security.
