The type of document we may need will depend on your individual circumstances but could, for example, be a copy of a diagnostic report or a letter from your doctor or specialist.
We can accept Form 8 Assessments/Letters confirming previous access arrangements for exam adjustments. You can obtain this from your school/college if you have not retained a copy.
