The type of document we need will depend on your individual circumstances but may, for example, be a copy of a diagnostic report or a letter from your doctor or specialist.
We can accept Form 8 Assessments for exam arrangements, but you will need to obtain this from your school.

If you don’t currently have a formal diagnosis, you may need to arrange (and pay for) additional evidence, such as an Educational Psychologist assessment for dyslexia.
We cannot recommend an assessor but do provide a list of organisations or professionals whose assessments meet our evidence requirements. You may also be able to apply for funding to recoup some of the costs of an assessment.
