The structures of all major complexes of the mitochondrial respiratory chain are now known at atomic resolution. Together with information from a wide range of functional biochemical and biophysical methods, these structures provide insights into the remarkable mechanisms that have evolved to convert the available redox energy into an electrochemical gradient of protons that can be used to power ATP synthesis. I will review progress and outstanding issues and highlight aspects that are relevant to health and disease.
