Foot and Ankle trauma

I receive tertiary referrals for complex foot and ankle trauma surgery from colleagues in Aintree and hospitals around the region. In the process, I have co-led on an improvement strategy in the treatment of ankle fractures. My work on ankle fracture treatment improvement has reduced the mal-reduction rate of 33% and 44% in audits in 2013 and 2015 to 2.9% in the audit in 2017. This has consequently reduced the major complication rate from 8% in 2013 and 10% in 2015 to 1.4%. This improvement has in part been due to our development of a complex ankle fracture protocol, which has developed into a classification system. This work has been presented nationally (British Orthopaedic Foot and Ankle Society 2015 and British Orthopaedic Association 2016) and has been resulted in a number of publications in Foot and Ankle International, Journal Bone Joint Surgery and the Annals Royal College of Surgeons. Using this classification, we have developed treatment algorithms for certain ankle fracture types. This protocol has now been in place for 3 years. On obtaining the patient reported outcome measures since the protocol has been in place, we have illustrated an improvement of 20% in functional results in 2017, compared to the results obtained from 2013. Our current results using the algorithms are better than any in the literature for these particular fracture types.
